    Sharing the development of a new 'in-house' tool which assists in the placement of individuals with complex needs and high risk behaviours. The SHEA could help to provide sustainable care and support with the prevention of hospital admissions; and assist with proactive planning at points of transition with due consideration of key issues (e.g. sensory needs).
